    三维彩色多普勒超声对子宫肌瘤与子宫腺肌症的鉴别诊断作用

    摘要 目的 探討三维彩色多普勒超声对子宫腺肌症与子宫肌瘤的诊断作用。方法 选取2017年12月—2019年12月于我院接受三维彩色多普勒超声诊断患者作为研究对象,其中子宫腺肌症(肌症组)和子宫肌瘤(肌瘤组)患者各50例,比较两组超声诊断结果,并与病理诊断比较,评价三维彩色多普勒超声对子宫腺肌症和子宫肌瘤的鉴别诊断效果。结果 内部血流检出率肌症组为84.00%,高于肌瘤组的40.00%,周边血流检出率肌症组为12.00%,低于肌瘤组的94.00%,差异具有统计学意义(P<0.05);肌症组动脉阻力指数(RI)2.93±0.85,搏动指数(PI)0.92±0.14,高于肌瘤组1.18±0.69,0.43±0.09;肌症组血流量(BFV)水平为64.16±10.58,低于肌瘤组的105.29±12.97,差异具有统计学意义(P<0.05);三维彩色多普勒超声对子宫肌瘤和子宫腺肌症的鉴别诊断灵敏度、特异度分别为93.00%和92.00%,并与病理诊断具有高度一致性(Kappa>0.75)。结论 在子宫腺肌症与子宫肌瘤的鉴别诊断中,三维彩色多普勒超声具有较高的真实性和与病理诊断的一致性,具有临床应用意义。

    Abstract  Objective To explore the diagnostic effect of three-dimensional color Doppler ultrasound on adenomyosis and uterine fibroids.Methods A total of 100 patients who underwent 3D color Doppler ultrasound diagnosis in our hospital from December 2017 to December 2019 were selected as the research subjects.50 patients with adenomyosis (myosis group) and 50 patients with hysteromyoma (myoma group) were enrolled in this study.The ultrasonic diagnosis results of the two groups were compared and compared with the pathological diagnosis.The differential diagnosis effect of three-dimensional color Doppler ultrasound on adenomyosis and hysteromyoma was evaluated.Results The diagnostic coincidence rate and misdiagnosis rate of the two groups were not statistically significant (P>0.05).The detection rate of internal blood flow in the myoma group was 84.00%, which was higher than 40.00% in the myoma group,and the detection rate of peripheral blood flow in the myoma group was 12.00%,which was lower than 94.00% in the myoma group. The difference was statistically significant (P<0.05); The RI 2.93±0.85 and PI 0.92±0.14 of the myosis group were higher than those of the myoma group 1.18±0.69, 0.43±0.09; the BFV level of the myosis group was 64.16±10.58,which was lower than the myoma group 105.29±12.97,the difference was statistically significant (P<0.05); The sensitivity and specificity of three-dimensional color Doppler ultrasound in the differential diagnosis of uterine fibroids and adenomyosis were 93.00% and 92.00%, respectively, and were highly consistent with pathological diagnosis (Kappa>0.75).Conclusion In the diagnosis of adenomyosis and uterine fibroids, three-dimensional color Doppler ultrasound has a high diagnostic coincidence rate and can effectively differentiate the diagnosis, which is worthy of promotion.

    综合上述,在子宫腺肌症与子宫肌瘤诊断中,三维彩色多普勒超声的诊断符合率高,且能够有效鉴别诊断,值得推广。
