B超引导下经阴道行卵巢囊肿穿刺术治疗不孕症伴卵巢囊肿的临床效果

[摘要]目的 研究B超引導下经阴道行卵巢囊肿穿刺术治疗不孕症伴卵巢囊肿的临床效果。方法 选取2017年12月～2018年12月聊城市第四人民医院收治的53例不孕症伴卵巢囊肿患者，经随机数字表法分为A组（26例）和B组（27例）。A组进行促排卵治疗，B组实施促排卵治疗+卵巢囊肿穿刺术，比较两组促排卵情况、临床效果、不良反应发生情况。结果 B组尿促性素（HMG）用量少于A组，B组用药天数、卵泡期天数均短于A组，B组妊娠率高于A组，B组的临床治疗总有效率高于A组，差异有统计学意义（P<0.05）。两组的感染、盆腔粘连等不良反应总发生率比较，差异无统计学意义（P>0.05）。结论 在不孕症伴卵巢囊肿治疗中用B超引导下经阴道行卵巢穿刺术，能减少HMG用量，缩短用药天数、卵泡期天数，提高妊娠率，临床效果显著。

Clinical effect of transvaginal ovarian cyst puncture under the guidance of B-ultrasound in the treatment of infertility with ovarian cyst

[Abstract] Objective To study the clinical effect of transvaginal ovarian cyst puncture under the guidance of B ultrasound in the treatment of infertility with ovarian cyst. Methods A total of 53 infertile patients with ovarian cyst were selected treated in the Fourth People′s Hospital of Liaocheng City from December 2017 to December 2018. By random number table method， patients were divided into group A （n=26） and group B （n=27）. Group A was treated with ovulation induction， and group B was treated with ovulation induction+puncture of ovarian cyst. Ovulation induction， clinical effect， adverse reaction between two groups were compared. Results The dosage of Urotropin （HMG） in group B was less than that in group A， the days of medication and follicular period in group B were shorter than that in group A， the pregnancy rate in group B was higher than that in group A， the total effective rate of clinical treatment in group B was higher than that in group A， the differences were statistically significant （P<0.05）. There was no significant difference between the two groups in the total incidence of adverse reactions such as infection， pelvic adhesion （P>0.05）. Conclusion In the treatment of infertility with ovarian cyst， transvaginal ovarian puncture under the guidance of B ultrasound can reduce the dosage of HMG， shorten the days of medication and follicular period， and improve the pregnancy rate. The clinical effect is significant.
综上所述，将B超引导下经卵巢穿刺术用于不孕症伴卵巢囊肿治疗，临床效果显著，患者用药天数、卵泡期天数明显缩短，HMG用量少，妊娠率高，说明这种治疗方式可临床推广应用。而受样本数量限制，以至于实验结果普遍性有所缺失，未来将搜集整理更多临床病例信息，再开展临床研究，得出更科学、准确实验结论，为后续同类研究工作开展提供理论支持。
