　 中医不传之秘在剂量

　 曾听到过一位资深望重的老中医告知：“中医不传之秘在于药量”，同一种中药，用量不同，产生的作用不同，甚至还可以产生完全相反的作用。中医治病的根本不是病，而是人，而每个个体的差异不同，用药剂量也会不同。但是圆机活法组方，巧在识证准确，用足剂量，何慢之 有（注：本段所提“两”为旧制，折合今为30克）

　例如：

　黄芪——其利尿作用在20克以内明显，30克以上就趋向抑制；其对血压影响，15克以内可升高血压，35克以上反而降压。若有气虚症状时，用炙黄芪，无气虚症状，则用生黄芪。生黄芪30g以上补气不助火，降压效果好，30g以下升阳助火。

　柴胡——在小柴胡汤中为君药，用量大于其它药一倍有余，起透邪外出的作用；在逍遥散中为臣药，用量与各药相等，起疏肝解郁的作用；在补中益气汤中为佐药，用量极小，起升举清阳的作用。

　白术——常用量能健脾止泻，大剂量用至30~60克，则能起益气通便的作用。

　红花——少用可养血，稍多则活血，再多则能破血。

　薄荷——用3克以疏达肝木，用至15克以发散风热，清利头目。

　桂枝——用量不到5克，可以起到温通阳气，增加膀胱气化功能的作用；用至10克，则有温经散寒，解肌发表，以祛除在表之风邪的效果。

　 川芎——小剂量可使子宫收缩加强，大剂量反而麻痹子宫。

　补充：

　【川芎 外感头痛，用量宜轻，最多不超过4克；高血压肝阳头痛，用量宜重，习用9~12克,；瘀血头痛，宜重剂量，可用至30~40克。

　历代认为是治疗头痛之要药。前人有谓“头痛必用川芎”。然头痛一症，病因殊多，川芎性味辛温，有活血行气、祛风止痛的功能，临床常用以治疗血瘀头痛。例如，

　王清任血府逐瘀汤治疗血瘀头痛，方中川芎常重用15～30g；清陈士铎《百病辨证录》散偏汤治偏头痛，疗效明显，方中亦重用川芎，用量达30g之多，若减少川芎的用量，则疗效不佳。若用川芎治高血压头痛时，亦应大剂量使用，可用10～15g。无论高血压或低血压所引起的头痛，只要是血中有滞，放胆使用川芎，不但止痛效果良好，而且对血压也有相应的调节作用。

　川芎引经少阳胜于柴胡，用量不宜多，一般在4.5~6g，治疗顽固性头痛时，剂量宜大，有效量在30g以上，最多可用至45g，配伍得当效果立竿见影！

　据近代药理研究认为，大剂量使用川芎能降低血压，小剂量使用能使血压上升。有人认为川芎辛温香窜，上行头目，高血压患者宜慎用。但中医认为本品有上行头目，下行血海的双向性作用。而且用川芎15克桑叶45克这样的剂量与配伍治疗血管性头痛有奇效。】

　山茱萸 固脱3两(90克)见功,必与参附搭配.

　穿山龙，味苦，性平，对细胞免疫和体液免疫均有调节作用，所以近年来成为治疗风湿类疾病的妙药。《中华本草》谓其干品用量为6～9克，《中草药手册》多为15克，少数达30克，东北地区常用量也为15～30克。但根据朱良春经验，若要取得较好的疗效，其用量须40～50克，用量在30克以下收效不明显。

　黄连最苦,然而治糖尿病这一甜病有特效,通常剂量为每日30克(30克是基本,配干姜以防伤胃),而治疗糖尿病酮症,每日剂量多达120克，可使降糖迅速.

　益母草 调经用10-15克。据朱良春观察，益母草的利尿作用，每日用到30～45克尚不见效，须加至60～75克，始奏明显之效。90～120克时其效更佳，常用以治疗急性肾炎之尿少、浮肿之候，常一剂知，二剂已。

　枳实 小剂量能使心脏兴奋，大剂量使之抑制。

　生杜仲：30g以上治腰痛奇效，不效则加至60－90g。

　萆薢、土茯苓：30g以上消除蛋白尿。

　沙参：30g以上补气，小剂量则可滑肠。

　薏米：30g以上治关节痛。

　茜草：30g以上治口疮。

　莱菔子、槐花：30g以上降压。

　生黄芪：30g以上补气不助火，降压效果好，30g以下起升阳助火的作用。

　代赭石：9～18 g有镇胃降气、止呕止噫之功，适用于胃气虚弱的呕吐、呕逆、呃气、胃脘满实等。24～30g用于治疗实证气喘及肝阳上亢所致头晕、目眩等症状。丹参大剂量可以治疗失眠。

　大黄 1～5 g有致泻作用。其致泻成分为葡萄糖甙元，番泻叶甙A、C，主要为蒽醌衍生物。3～6g可止泻，9～15g可泻下；两许--疔毒之毒热甚盛者二两--癫狂其脉实者--医学衷中参西录,治疗肝炎，随用药量增加而各项指标复常时间缩短，所以认为30g可作为常规剂量。大黄粉0.3g以下有止泻作用。其机理为大黄鞣酸的收敛作用掩盖了含量甚少的致泻成分的作用。鞣质的D-儿茶精抑制大肠内细菌生成酶，阻断吲哚类的产生而止泻作用。

　白果 定喘汤白果用量在21枚（约为25g左右），动物实验证实，定喘汤中白果重用的定喘效果优于常规剂量。

　艾叶：3～5g可开胃,8g左右温经止血、止痛。而大剂量可使肝细胞损害，出现中毒性肝炎；另外，大剂量还会引起胃肠道炎症。

　槟榔 ：用以消积、行气、利水。常用剂量为6～15g,当用以杀姜片虫、绦虫时，即须用到60～120g。

　苍耳子 少量则轻而上至颠顶,重用则通下走足膝。

　细辛 治疗风寒表证的剂量一般用3克，最多不超过9克。而当用细辛治疗各类痛证时，用量常常超大，有时用至30克，甚至更大。显然，当风寒表证时用超大剂量的细辛，不仅于证无益，而且还会引起不良反应；同样，如果要发挥细辛的镇痛作用，而用常用剂量，显然是杯水车薪，于痛无济。

　薏苡仁 系药食两用中药，其常用剂量的上限为30克，而临床上有经验的医师用该药治疗风湿、腰腿痛等病证时，该药的用量达到45～90克。

　夏枯草 常用剂量的上限是15克，而临床以该药治疗病程较长的甲状腺瘤时，用量一般都超过30克。

　三棱 为破血行气之药，常用剂量的上限为9克，但临床上以该药配合其他中药主治各类晚期恶性肿瘤病时，其每日用量达到45～75克，相当于权威规定剂量上限的5～8.33倍。

　当代名医方药中教授深得《金匮要略》用升麻之真谛，临床重用升麻治疗病毒性肝炎，也是取其解毒之偏性《药典》中规定黄芩的每日剂量为3～9克，川芎为3～9克，而《千金翼方》中以单味黄芩治疗淋、下血诸症，黄芩的剂量为每日四两，折合公制为57.68克，显然已经大大超出黄芩权威规定剂量的上限，剂量一般为每日30～45克。

　《小品方》中以单味川芎治疗妇人崩漏，每日剂量用至八两，折合公制115.36克，也明显是属于超大剂量应用。

　人参的常用剂量是3～9克，当用于脱证时，其用量高达30克以上。

　枳实——小剂量能使心脏兴奋，大剂量使心脏抑制。

　生杜仲――30g以上治腰痛奇效，不效则加至60－90g。

　薏米――30g以上治关节痛。

　茜草――30g以上治口疮。

　两药用量配伍不同其作用也有所不同

　例如：许公岩治疗积湿的病症，以苍术、麻黄二药为主。当两药相等时，剂量是： 10g：10g，临床常见能发大汗；当苍术倍于麻黄时，则小发汗；当苍术三倍于麻黄时，剂量是：18g：6g，常见尿量增多，有利尿之作用。当苍术四倍于麻黄时，剂量是： 12g：3g，无明显之汗利，而湿邪能自化。

　萆薢、土茯苓――30g以上消除蛋白尿。

　沙参――30g以上补气，小剂量则可滑肠。

　疗效的好坏，关键在于药物之间的比例。并非药量越大，疗效越好

　>>>再如治妇人胎前产后气郁血瘀诸疾的佛手散，当归用二至三两者，乃取其活血之用，使瘀去新生、血有所归。由此可见，当归用于活血，剂量宜大，可用至15g以上。前人谓其气味俱厚，行则有余，守则不足，故重用则行血之力更甚。若当归用于补血，剂量宜轻，3～9g即可。血虚者每致阴虚，阴虚则生虚热。当归气味辛温而主动，重用则每致动血，切不可重用，否则适得其反，病家服后每致口干、烦躁、失眠、头晕更剧，甚则鼻衄。

　中药超大剂量应用的注意事项

　 中药的超大剂量的应用属于临床中药学研究范畴的内容，这方面的研究目前还只是刚刚起步，许多内容还是未知数。目前尚不能明确地回答每味中药在什么条件下可以超大剂量应用，剂量超出权威规定的剂量上限多少时最适宜，超大剂量应用中药是否对机体有潜在的不良反应等问题。

　中药超大剂量应用的主要依据仍然是古代及现代医家的临床用药经验，而现代中药药理、毒理学研究的结论却尚未用于临床指导中药的超大剂量应用。在目前的这种情况下，对中药的超大剂量应用应持谨慎态度。中药超大剂量具体应用时，必须注意下列有关事项。

　一、适应症要准确

　 中药的超大剂量应用都有相应的适应症，超大剂量用药，适应症一定要准确，否则，会出现两种结局：一是药重病轻，二是药轻病重。例如，中药细辛治疗风寒表证的剂量一般用3克，最多不超过9克。而当用细辛治疗各类痛证时，用量常常超大，有时用至30克，甚至更大。显然，当风寒表证时用超大剂量的细辛，不仅于证无益，而且会引起不良反应；相反，如果要发挥细辛的镇痛作用，而用常用剂量，显然是杯水车薪，于痛无济。

　三、严格炮制、制剂规范

　 许多中药在超大剂量应用时，在炮制、煎药和制剂技术上有比较严格的规定性，中药在这方面的经验比较成熟，应用时应该严格遵守。附子、乌头类中药超大剂量应用时，特别强调先煎、久煎，以煎煮至不麻口为标准。现代药化、药理研究结果证明，上述炮制、制剂规范是非常必要和合理的，它能够保证在不影响药效的前提下，有效地降低这类中药的毒性。

　四、特异性的配伍

　 从古籍和名老中医超大剂量应用中药的经验中，我们还可以发现某些中药超大剂量应用时，常有一些比较特定的配伍关系。如半夏配生姜、甘草；马钱子配甘草等。其目的是防止和降低超大剂量用药可能引起的毒性及不良反应。临床应用时必须严格遵循。
